Background Check Paperwork
Girl Scouts of Southeastern Pennsylvania

All parents, please complete the background check and reference forms in addition to your registration forms. This allows us to have you chaperone trips and drive. This may also be acceptable to other agencies that require background checks, such as schools and church groups. (Almost all groups that deal with children are required to get background checks by their insurance companies now. The cost for the GSSP background check is $5, the best price I have seen. -E.C.)

Reference forms - please print double sided, so you can use the postage paid reply printed on the back. Print two double-sided forms, for two different people you choose as references.
